The Riverside Health System Foundation helps fund public-private initiatives that support the Foundation’s mission.

There are several features that the Riverside Health System Foundation believes are essential for a strong project. The Foundation looks for these in every sponsorship application it reviews. Overall, a successful application must demonstrate that all necessary planning has taken place and project management is structured to begin implementation.
Concept Paper
Concept Paper Guidelines:
- No more than three typed pages in length
- Project Director’s name, title, address, phone and fax number
- A brief background statement about the applicant organization and its mission
- A brief description of the problem
- A brief description of the project, the population to be served, the project outcomes expected and how the outcomes will be measured.
- Total amount of funding desired
- Budget
Please submit a your concept paper for the second grant cycle via e-mail to Holly Prinzel, Development & Program Manager, at holly.prinzel@rivhs.com, by July 7, 2008.
Letters of invitation to submit a final grant proposal will be sent within 10 business days of the concept paper deadline.
Full Grant Proposal
A successful application will:
- Address an identified community need
- Improve the quality of life in the community served by targeting a clearly defined under-served population or one not previously served
- Be innovative, proactive new and need seed money or are established, effective, and need continued funding.
- Demonstrate that at least 25% of the project/program or initiative cost are/will be underwritten by the community. Also, the requesting agency or organization will need to present a viable plan to sustain funding after the Riverside Health System Foundation’s committed funds are depleted.
- Reflect strong organizational/agency leadership and broad based community commitment and collaboration.
- Agree to participate in the Riverside Health System Foundation’s evaluation of measurable outcomes and continuous review of the project/program or initiative.
Please note: The Riverside Health System Foundation will NOT:
- Make grants available to individuals
- Make grants available to organizations/agencies that are not tax-exempt under 501 c(3)
- Make grants available to political action or legislative advocacy groups
- Make grants available to organizations that discriminate by race, color, creed, gender, or national origin
- Make grants available to endowment funds
- Replace funding from a previously-existing source, either public or private, which has been reduced or eliminated

Word 6.0/95). There are two grant cycles annually. Applications will be due at the Foundation’s office by 4:30 p.m. on the deadline date as listed above. The burden is on the applicant to determine if the Grant Proposal Application has arrived by the deadline. Grant Proposal Applications received after the deadline will be held for review during the next grant cycle. The Foundation requires one original and two (2) copies of the Grant Proposal Application and Attachments, as well as the completed Application in Word or Rich Text Format. If the due date falls on a Saturday, Sunday or a holiday, then it is due the next business day by 4:00 p.m.
Conditions for Grant Acceptance - Each grantee must sign a Memorandum of Understanding with Riverside Health System Foundation, which delineates the terms and specific objectives of the project/program or initiative.
- Each grantee must agree to submit requested data and reports on a timely basis and assist with the evaluation process as requested.
Organizations are responsible for assuring receipt of their proposal in the Foundation office on or before the deadline. Please send one original and two (2) copies of the Sponsorship Application and Attachments, as well as the completed Sponsorship Application in Word or Rich Text Format:
